目的 中心静脉插管的儿童发生可疑血液感染时， 常常需要花费48 h的住院时间以等待血培养结果， 同时根据经验给予抗生素的治疗。 相对于传统的血培养方式， 连续监测血培养系统较此前的血培养系统能更早地观察到可能存在的血流感染， 以便决定早期的诊疗决策， 从而缩短根据经验给予的儿童抗生素治疗和住院治疗时间。
方法 本次回顾性队列研究纳入了在门诊就诊的、 经实验室检查确诊存在血流感染的中心静脉留置导管患儿。
结果 有200例次血流感染被纳入本次研究。 患儿的中位年龄为5.5岁， 中心静脉插管的中位时间为80.5 d。 革兰阴性细菌感染率为51%， 其中25%为单一细菌感染， 26%含有 ≥2种的细菌感染。 总体而言， 从采血培养到报告阳性结果的中位时间为14 h。 革兰阴性细菌感染36 h时培养阳性的预测概率为99.2%； 对年龄、 插管类型、 近期使用抗生素等因素进行校准后， 36 h时的培养阳性率为96.6%。 在多变量Cox相对危险度回归模型中， 校准年龄、 导管类型和近期使用抗生素等因素后， ≥1种革兰阴性菌多重感染和单种革兰阴性菌感染与经年龄、 导管类型和近期使用抗生素等因素校准后的血培养得出阳性结果所需时间较早有关。
结论 血培养得出阳性结果所需的时间与细菌种类有关。 革兰阴性细菌导致的血流感染通常能很快地被检测出来。 我们的研究揭示了对采血后24 ～ 36 h时血培养仍然阴性、 病情稳定的中心静脉插管患儿， 停用根据经验给予的抗生素治疗在临床上是安全的。关键词
